Effective 1st November 2016, E-form with Biometric Device (Electronic Thumbprint Verification) is compulsory for TM customer who wish to register Telephone Line, Streamyx and Unifi.

Residential applicant need submit a clear photocopy IC (Front & Back) of customer for Malaysian or a clear photocopy passport of customer for foreigner (Passport expiry date must not be less than 2 years from registration date).

Commercial (Business) applicant are required to submit the SSM documentation with the “Certified True Copy” (CTC) as a supporting document. We are unable to process business order for Unifi and Streamyx if you fail to provide SSM documentation with CTC
TM will only accept SSM documentation with CTC:

- Certified by Company Secretary, or
- Certified by Commissioner for Oaths (Pesuruhjaya Sumpah), or
- Certified by Other Idendified Authorities, or
- Digitally certified by SSM online. You may advice customer to download and print the SSM documentation with CTC at the link below:

Supporting documents required for Maxis Home Fibre package:

- Application form with customer signature (Printed copy or original copy)

- A clear photocopy IC of customer for Malaysian

- A clear photocopy passport for foreigner must be valid at least 6 months from the date of registration

     
Foreigner must pay upfront deposit of RM500 (Foreigner Deposit) before Maxis can proceed with the registration.
After submit the order, we will issue FTTH Account number immediately, then customer need pay RM500 foreigner deposit at Maxis centre Only within 48 hours. Please show Maxis centre the FTTH account number and verify the name.
After making payment, please scan and email the payment slip to us.

Order will be rejected if customer did not make payment within 48 hours after the FTTH account number is generated.
